What’s the difference between mg and mcg?
One milligram equals one thousand micrograms. That’s the whole relationship: 1 mg = 1000 mcg. A milligram (mg) is a thousandth of a gram; a microgram (mcg, sometimes written µg) is a thousandth of a milligram. So mcg is the smaller unit — it takes a thousand of them to make a single mg.
Why this matters for peptides: most peptide doses are tiny, so protocols are usually written in mcg, while the vial label almost always states its total in mg. A vial says “5 mg” on it; your protocol says “250 mcg.” Those are the same scale of measurement — you just have to keep the units straight when you do the math.
What is the 1000× dosing mistake?
The 1000× mistake is reading a dose in the wrong unit — treating 250 mcg as 250 mg. Because mg is a thousand times bigger than mcg, mixing them up doesn’t make you a little off. It makes you off by a factor of 1000. There’s no small version of this error.
It happens because the numbers look identical. “250” reads the same whether it’s followed by mg or mcg, so it’s easy to glance at a protocol, see a familiar figure, and skip the unit entirely. The decimal slips, the unit gets dropped, and the number that survives is the dangerous one.
How do I confirm whether a dose is in mg or mcg?
Read the unit out loud, then check it against the vial label before you touch a syringe. The protocol unit and the vial unit have to agree once you convert them — if they can’t be reconciled, stop and re-read, don’t guess.
- Find the dose unit in your protocol. Is it written in mg or mcg? Say it out loud — “two hundred fifty micrograms” — so the unit registers, not just the number.
- Read the total on the vial label. It’s almost always in mg (e.g., “5 mg”). This is the total amount of peptide in the whole vial, not your per-dose amount.
- Put both in the same unit. Convert your dose to mg by dividing mcg by 1000 (250 mcg = 0.25 mg), or convert the vial to mcg by multiplying mg by 1000 (5 mg = 5000 mcg). Pick one unit and stay in it.
- Sanity-check the size. Your per-dose amount should be a small fraction of the vial’s total. If your “dose” is equal to or bigger than the whole vial, you’ve mixed up a unit — go back to step 1.
How many doses are in a vial?
Doses per vial = the vial’s total in mg ÷ your dose in mg. The only trick is making sure both numbers are in the same unit before you divide — which is exactly where the mg/mcg slip bites people.
Worked example: a 5 mg vial dosed at 250 mcg per dose. First convert the dose into the same unit as the vial: 250 mcg ÷ 1000 = 0.25 mg per dose. Then divide: 5 mg ÷ 0.25 mg = 20 doses in the vial. That’s it.
- Vial total: 5 mg
- Your dose: 250 mcg = 0.25 mg (divide by 1000)
- Doses per vial: 5 ÷ 0.25 = 20 doses
Notice what would happen if you forgot to convert. If you divided 5 mg by 250 (treating the dose as 250 mg instead of 0.25 mg), you’d get 0.02 — telling you the vial holds a fiftieth of one dose. That nonsense answer is the unit error showing itself. A clean answer should be a sensible whole-ish number of doses; a bizarre one means recheck the units.
How do I make sure I never slip a unit?
Build a few fixed habits so the unit is checked the same way every time, instead of relying on catching it in the moment. The mistake hides in haste and autopilot — a routine removes both.
- Always write the unit, never just the number. “250 mcg,” not “250.” A bare number is a future mistake waiting to happen.
- Convert everything to one unit before you do any math. Pick mg or mcg and put both the vial and the dose into it first — don’t divide across two units.
- Re-read the vial label against your protocol every time you draw, not just on day one. Vials and protocols don’t always use the same unit.
- Treat any weird-looking answer as a unit error first. A dose bigger than the vial, or a vial holding a fraction of one dose, almost always means a mg/mcg mix-up — not a bad vial.
